Primary Care Management (PCM)
Principal Care Management (PCM) is a critical component of primary care that contributes to better health and care for individuals. PCM is for patients that have a diagnosis expected to last between three months and a year, or until the death of the patient, may have led to a recent hospitalization, and/or place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.
